Random Post

Unfortunately, the passover didn't go as well as I hoped.
I answered the question that I was asked to present, but I was then asked on how LAs actually work and about it's toxicity. So, that is my homework for today - to present tomorrow. I don't really mind actually, at least I will learn something.

I am oncall tomorrow.
Every call is a stress.
Every call is a challenge.
Every pre-call is a depressing day.

Anyway, I heard Ipoh was quite a happening place today.
I have no idea what happened.
I have no idea who is the MB, who is the government.
Our day in the hospital is no different.
Admissions, clerkings, discharges.
Life goes on as usual.

Just the other day, there was a news article about a 6 year old boy who probably died of Paracetamol (PCM) overdose.
Supposedly, the doctor prescribed 2 tablets of PCM to be taken 4 times a day.
I wonder how such a mistake could happen.
A 12 year old would probably generally know that kids take 1 tablet of PCM while adults take 2.
(As a doctor, you calculate the amount based on the child's body weight - 15mg/kg)
They have set up an inquiry for this case for now - let's see what happens next.

Have to study my LA now.

Today, I learnt that carbon dioxide in the blood stimulates the respiratory system - the carbon dioxide drive; which means, that the reason we breathe is because of high carbon dioxide level in our blood stream.

However, as we live in a polluted world, by the time we get old, our body is almost resistant to carbon dioxide drive (I don't know why) and we depend on oxygen drive - which means that when there is low level of oxygen in our body, we breathe in more.


When someone has trouble breathing and we give them oxygen, then the oxygen drive does not work (as there is no hypoxia). And so, if we prolong the supplementation of the oxygen, we are basically not helping the patient as we are surpressing their oxygen drive.


(p/s the things that I learnt may not be very accurate, it is just what I understand from what I hear/read.

A New Thing A Day

I decided that I should take the effort to learn something new everyday.
It can be anything - related to work or just a general thing.

A new thing a day, or at least a new thing every blog entry.

Since it is a little late right now, and I am kind of sleepy, I am not going to think very hard about what to write for today.

I was given a 'homework' by one of my specialists that I have to present during passover tomorrow. That will be my 'new thing for the day'.

Today, I found out why local anaesthetic(LA) doesn't work if the area of interest is inflamed/infected.Justify Full
LA is a base. Inflamed tissues are acidic.
So, as the LA is neutralized, there won't be sufficient amount of LA to penetrate the tissue and produce it's effect. (something like that la)

Hopefully it will go well during passover tomorrow..

Just Another Scratch

I was postcall on Saturday.

After work, I drove to KFC, tapau-ed my lunch, went back home, ate then slept.

The next day, I walked towards my car and this is what I saw.


Who did it?
I have no idea.

When did it happen?
I don't know.

What can I do about it?
Nothing.

Life goes on.
As it is I have another scratch on the other side of my car (my fault).

I HATE IT!!!!

26/04/09 - Pre-call (have to go to the ward that I'm going to be oncall in to get to know the cases there
27/04/09 - Oncall
28/04/09 - Post-call
29/04/09 - Pre-call
30/04/09 - Oncall
01/05/09 - Post-call (get to go back half day cos of public holiday)
02/05/09 - Passive call (since it's a weekend, I get time off from the end of specialist rounds up to about 7pm, but have to work until about 11pm).
03/05/09 - Oncall

What the %#@*!!

I HATE this department.
So far, I have not found anything that I like about this department.

Every morning, I have to go to work early just to try to finish the rounds.
Passover starts at 7.30am, and usually the MO rounds start right after passover.
Which means, your rounds have to finish before 7.30am.
On top of that, if you clerked any cases the day before that, you have to find the tickets of those patients to bring downstairs for passover.

Passover - if you clerked the case, you present the case in front of all the specialists and MOs and HOs.
If the specialists have any questions (regarding the patient) for you, they ask you.
If you can't answer, you are an idiot 'cos that is your patient.

So many times in Paeds, we, HOs were told that we are not clerks, but doctors.
But here, I feel like I am a clerk.
Nothing more.

So many times, I feel like I am unwanted in this department.
I feel like everyone hates me (and vice versa).
My colleagues are OK though, most of them are nice.
The specialists and MOs - the moment I feel that one of them is nice (or not bad), they will say something to make me hate them all over again.
ARGH!!

Everyday, there is a stress while going to work.
So far, I have not dreaded going to work.
But I think, soon, that is going to change.
I HATE ORTHO!!!!
I hope I am not extended.
I don't want this torture to be prolonged!
Already I am just waiting to get out of the department.

Enjoying Myself

It's been a long time since I updated my blog.
I have had quite busy this week.
My MO and specialist were not happy that we (HOs) are not finishing our rounds before they start their rounds, so, now, we are forced to start our rounds early, at about 6.30am.
That, plus the fact that there are so many things I don't know about Ortho, which means I have to come back and study everyday makes blogging even harder.
However, what totally stopped me from blogging recently is a story book.

There are many kinds of story book.
Obviously every loves a feel-good kind of story book.
Nobody wants to feel sad after reading a book (or watching a movie for that matter).
It's the same with me.

There are story books which make you think, make you appreciate what you have.
There are story books which make you feel like puking.
There are books you wish you never started reading.

The book that I got me stuck this time is a simple book.
A book who's ending was very predictable.
No complicated storyline.
No hard, un-understandable words.
But I really enjoyed reading it.
Maybe 'cos it has been such a long time since I read such a book.
A book that you can just read and enjoy.
A book you look forward to reading.
I miss those times when I didn't have to care what time I slept - just read until you are tired enough to sleep 'cos you can always catch up with sleep the next afternoon.

My short break is over.
Time to get to studying.
I am oncall tomorrow.

It's A Small World

Just the other day, a friend of mine suddenly told me - 'Hey, you know something, we are related!'

One thing about Indians (I don't know about Malays and Chinese and everyone else so I won't comment on them) but Indians, when they know you are from A, they will ask you do you know so-and-so who is also from A?

I don't know how many times people have asked me whether or not I know this person (or that person) who also studied in Russia.
Most of the time, my answer is no, because I am an anti-social person.
Plus, there are at least 5 universities that Malaysians go to, and there are at least about 500 Malaysian students in each university (no, I am NOT exaggerating).

Anyway, this girl (SG) she went for a wedding and met her cousin's wife (AF) there.
The wife then asked her whether or not she know Sharini (since SG said she is working in Ipoh).
SG said yes, 'cos we were together in Paeds.
And then, AF told her that I am her cousin.
See how the connection goes?
So, basically, me an SG - we are distantly related.

This just goes on to show how small the world really is!
(I am sure you would have had similar episodes too!)
I may be related to you, for all you know (you can be an Indian, Chinese or Kadazan - I have them all in my family tree).
